Query by email only, no postal mail.  Do not send attachments.  Include "Author Query" in the subject line to circumvent spam filters.  Add your name and the title of your work.

Direct emails to Elizabeth Griffin. (You may add "ATTENTION to" for a particular agent.)

Fiction Email query:  the genre, description of your submission, word count, author information, and the first four pages of your manuscript in the body of the email. 

Non-fiction Email query:  the genre, table of contents and author biography, including relevant experience and qualifications, published works, awards, etc..

Additional information

Mr. Dean Cooke left Bantam/Seal Books in 1992, giving up his position as publisher, in order to found The Cooke Agency.  He has worked in publishing for more than thirty years.
